It is important to remind you that benefit elections are generally binding for the entire year and cannot be changed until the next Open Enrollment period, unless you experience a qualifying event that allows for benefit changes during the year. If this happens, you MUST notify our Administrators at The Capitol Group of such a change within 30 days of the qualifying event date.
All employees need to complete the following Annual Election Form.
If you are enrolling in any insurance plan, please complete the benefit election form AND the corresponding insurance company enrollment form for that specific benefit election. You will find those forms, to download and print, on the site page associated with that benefit. Each plan has its own form.
